Abstract

Pencampuran part asing (foreign part mixing) antara part KOJA dan KIZA pada proses Heat Treatment Quenching Tempering (Q&T) di PT XYZ merupakan kegagalan kritis yang mengakibatkan 8-11 kasus komplain pelanggan per bulan. Kedua part memiliki kemiripan dimensi tinggi (look-alike parts) dan diproses pada mesin Q&T yang sama secara bergantian, sehingga risiko carry-over antar-lot sangat tinggi. Penelitian ini bertujuan mengidentifikasi failure mode menggunakan FMEA, memprioritaskan risiko melalui analisis Pareto, menganalisis akar penyebab dengan Fishbone Diagram 4M, serta menyusun rekomendasi perbaikan berbasis Poka-Yoke dan 5S. Sepuluh failure mode teridentifikasi dengan nilai Risk Priority Number (RPN) tertinggi pada proses penerimaan/unloading (RPN=324), penyimpanan WIP (RPN=270), ketiadaan traceability (RPN=252), dan carry-over Q&T (RPN=224). Analisis Pareto menunjukkan empat failure mode dominan berkontribusi 79,4% dari total RPN. Rekomendasi perbaikan meliputi implementasi barcode scan dua titik (gate-in dan gate-out), checklist changeover Q&T berbasis foto dengan tanda tangan operator dan leader, zoning area WIP dengan kode warna merah-biru, dan verifikasi dua orang pada outgoing quality gate. Simulasi menunjukkan total RPN turun dari 2.276 menjadi 340 (penurunan rata-rata 85,1%) dengan proyeksi komplain turun dari 8–11 menjadi 2-4 kasus per bulan.
