Abstract

Deteksi dini depresi di layanan primer membutuhkan instrumen skrining yang akurat sekaligus praktis. Tinjauan sistematis ini membandingkan akurasi diagnostik Patient Health Questionnaire-2 (PHQ-2) dan PHQ-9 untuk skrining depresi. Pencarian dilakukan di Google Scholar dan PubMed dengan kata kunci terkait (publikasi 2020-2025). Seleksi menggunakan alur PRISMA dengan kriteria inklusi studi komparatif pada dewasa di layanan primer. Lima studi (6.500 subjek) dari berbagai populasi memenuhi kriteria. PHQ-9 menunjukkan sensitivitas lebih tinggi (0,88-0,90) dibandingkan PHQ-2 (0,75-0,80), dengan spesifisitas masing-masing 0,85 dan 0,78. PHQ-2 efektif untuk skrining awal dengan administrasi lebih cepat, sedangkan PHQ-9 memberikan evaluasi komprehensif untuk diagnosis definitif. Pendekatan bertingkat mengoptimalkan deteksi dini dengan menggunakan PHQ-2 sebagai skrining awal dilanjutkan PHQ-9 untuk evaluasi mendalam. Disarankan menggunakan pendekatan dua tahap (two-step screening): PHQ-2 untuk skrining cepat semua pasien, lalu lanjutkan PHQ-9 pada hasil positif untuk konfirmasi.
